The Community Health Worker (CHW) will be responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors.
As a priority, activity will promote, maintain, and improve the health of clients and their family.
Provide social support and informal counseling, advocate for individuals and community health needs.
Community outreach, such as home visits will be required.
Help Me Grow is Ohio's evidenced-based parent support program that encourages early prenatal and well-baby care, as well as parenting education to promote the comprehensive health and development of children.
Help Me Grow System includes Central Intake, Help Me Grow Home Visiting and Help me Grow Early Intervention.

The individual in this position will work with the Help Me Grow program.
Help Me Grow is a system of supports for pregnant women, caregivers with new babies, and families with young children with developmental delays and disabilities.
This position will require home visits with clients.
This position may require irregular hours.
This position will also require travel by the employee in his/her own vehicle.
Qualifications:
Associates Degree, required.
Bachelor's Degree, preferred.
Successful completion of a Community Health Worker formal training program such as from a college or other education institution.
Verifiable good driving record and reliable transportation Job Type:
