Licensed Social Worker (LSW) or Registered Nurse (RN) Details:
LSW's will have interactive contact with, individuals and/or their caregivers by phone and in-person visits.
Case Managers will coordinate and monitor the delivery of necessary services with other services in the individuals service plan to enable clients to remain in the community living arrangement.
Traveling about 50% of the time and completing visits within 2 counties of the surrounding 9 counties (Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca and Wyandot) with mileage reimbursement.
NO EVENINGS, NO WEEKENDS, NO 'ON-CALL' time Knowledgeable in Word & Excel Must have LSW holding an active license to practice in the state of Ohio Perks:
